Insurance Information
We accept most major insurance plans.
Appointments
Please call the office to make an appointment in advance. If you are unable to keep your appointment, please call us as far in advance as possible so we may use that time to see another patient in need of care. We make a sincere effort to adhere to our appointment schedule and appreciate your patience if we are late due to emergencies or hospital surgery.
Fees & Payments
We are contracted with most major insurance companies and we will be happy to bill them. Patients will be responsible for any remaining balance that is not paid. Copayments are to be paid at time of visit. We accept cash, checks, and all major credit cards. Prescriptions & Renewals
If you have a prescription that needs to be refilled, please call the pharmacy that last filled your medication at least 24 hours before you will need the medication. If you have a new prescription need, please call our office during business hours.
What to Bring to Your Visit
- Your insurance card or other health information.
- Some insurance carriers require you have a referral or primary care authorization before treatment is given. Please check with your carrier.
- X-rays, MRI's and/or reports if available.
- A pair of gym shorts for knee and hip exams.
- A tank top or sleeveless shirt for shoulder exams.
Privacy Notice
Proliance Surgeons – Evergreen Orthopedic Center respects your privacy. We understand that your personal health information is very sensitive. We will not disclose your information to others unless you tell us to do so, or unless the law authorizes or requires us to do so.
The Notice of Privacy Practices describes in more detail how your health information may be used and disclosed, and how you can access your medical information. When you visit us, you will be asked to sign an acknowledgement that you have been informed of the Notice of Privacy Practices.
